For each statement below, think about how often you've felt this way over the past two weeks and choose one of the options:
0 = Not at all | 1 = Several days | 2 = More than half the days | 3 = Nearly every day
- Little interest or pleasure in doing things
- Feeling down, depressed, or hopeless
- Trouble falling or staying asleep, or sleeping too much
- Feeling tired or having little energy
- Poor appetite or overeating
- Feeling bad about yourself — or that you're a failure or have let yourself or your family down
- Trouble concentrating on things (like reading, work, or TV)
- Moving or speaking so slowly that others notice — or being so restless you can't sit still

Scoring (rough guide)
Add up your scores from 0–27:
- 0–4: Minimal or no depression
- 5–9: Mild symptoms
- 10–14: Moderate depression — worth discussing with a therapist or doctor
- 15–19: Moderately severe — professional support strongly recommended
- 20–27: Severe — please reach out for help soon
